Community Connector
Location: Reading, Berkshire
Hours: 28hrs per week
Contract Type: Permanent
Salary: £22,781 per annum based on 35 hours per week (£18,224 per annum at 28hrs per week)

Are you an enthusiastic and diverse individual? If so, read on to join our vibrant Health & Care team in a new service being delivered in partnership with Reading Borough Council.

Supporting people to build sustainable connections with local services that are aligned to their goals and interests can reduce loneliness and isolation, increase community connectedness, and improve people's sense of wellbeing.

Utilising your experience of delivering person-centred support, you will assist people who contact adult social care to identify their personal assets, interests and goals. This will enable them to increase and maintain their wellbeing, boost independence, become more resilient, finding solutions and support in their local community. You will also recruit, induct and assist a small cohort of volunteers who offer an invaluable contribution towards delivery of the service.

What does a day in the life of a Community Connector involve?

  • Providing person-centred assistance to local residents via the telephone or face-to-face in the community.
  • Working collaboratively as part of the Local Connectors team and a wider multidisciplinary team of adult social care professionals.
  • Building connections with local voluntary sector organisations to gain knowledge of the services they provide, and link local residents with them.
  • Using a range of IT solutions to capture case notes, identify support options, and communicate with colleagues and partners.
  • Providing day-to-day assistance to volunteers to enable them to support service delivery.

What do you need to be a successful Community Connector?

  • Experience of case work, supporting vulnerable people
  • Experience of person-centred support planning, helping individuals to identify goals and achieve positive outcomes
  • Confidence in using IT software, including Microsoft and relationships management systems, e.g. Mosaic.
  • Excellent time management and the ability to prioritise work and manage a proactive and reactive workload.
  • Strong interpersonal skills, relationship building, active listening with a wide audience.
  • Integrity, empathy, compassion and emotional intelligence.
