About The Role

Service Support Worker (Home from Hospital Service)
Location: Croydon, London
Salary: £22,932 (plus Inner London Weighting £3,344 if applicable) per annum pro rata
Hours: 28 per week
Contract: Fixed Term (until end of June 2026 with possibility to extend)
Driving: 
A full driving licence and access to a vehicle is preferred but not mandatory

What will a day in the life of a Support Worker involve?
  • Providing practical and emotional assistance to people returning from hospital or medical facilities. 
  • Accompanying service users to attend hospital appointments and other activities.
  • Liaising with health, housing and other professionals to ensure that the needs of the service user are being consistently met.
  • Ensuring a safe living environment for service users by undertaking risk assessments. 
  • Carrying out welfare checks to assist service users with day-today activities. 
  • Building positive relationships with service users, their families and other healthcare professionals.
  • Ensuring all care documentation is accurate and up to date.
What does it take to be a successful Support Worker?
  • Previous experience in a similar care, preferably within a healthcare or social care setting.
  • Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals’ lives.
  • Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
  • Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
  • Ability to work independently, demonstrating initiative and reliability.
  • Flexibility to work various shifts, including evenings, weekends, and bank holidays.
  • A full driving license and access to a vehicle is preferred but not mandatory.
