Community Connector
Location:  Basingstoke GP Surgery and Hampshire area for the B-Connected Network
Contract: Fixed Term Contract until 3rd May 2022
Hours per week: 35 hours  (Monday to Friday between 9am and 5pm, flexible)
Salary: £20,394 per annum (pro-rata)

We have a fantastic opportunity for a motivated and proactive individual to join our Independent Living team as a Community Connector covering Basingstoke.

Hosted by the British Red Cross on behalf of the Primary Care Network, the Community Connector will be within a Patient Support Team remotely as required or in the member practices. Reporting to the PCN Care Coordinator, you’ll be working with those affected by long-term physical health conditions, physical inactivity and social isolation to help them gain confidence, self-manage their conditions and reconnect to their community.

 The Independent Living (IL) & Crisis Response (CR) service (IL/CR) supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care, and emergency services.
 
 
This role could be for you if…
 
 To be successful in this role you must have knowledge of the local area served by the scheme and have following skills and experience:
 
  • Ability to work as part of a team and on own initiative
  • Able to plan own workload
  • Educated to GCSE level (or equivalent by experience)
  • IT literate
  • Have understanding of how to improve service quality for the benefit of users
  • Car user driver (essential)
  • Desired NHS background
  • EMIS user would be an advantage
  • Willingness to work flexible hours
