About The Role

Service Coordinator - Tri-Borough
Location: Chelsea and Westminster, Charing Cross and St Mary’s hospitals, London
Salary: £23,590 to £25,155  (plus Inner London Weighting, £3,344, if applicable) per annum
Hours: 35 per week
Fixed Term Contract (until 30th September 2026)

What will a day in the life of a Service Coordinator involve?
  • Meeting with our service users and assessing their needs, guiding or signposting them to connect with groups or to manage a health condition. You'll enable them to lead independent lives and improve their wellbeing.
  • Liaising with partnership agencies to promote the service and build meaningful links.
  • Producing and maintaining monthly reports.
What do you need to be a successful Service Coordinator?
  • Good communication skills.
  • Sound knowledge of Health and Social Care services.
  • Proven ability to deliver high quality public services.
