Service Coordinator
Location: Portsmouth and surrounding community
Salary: £22,781 - £24,244 per annum pro-rata
Hours: 30 per week - Core hours of work: 08:30 – 15:30. Monday-Friday, flexible working considered
Contract Type: Fixed Term Contract Until 30th March 2025

The preferred candidate must be able to travel across the area and have previous experience in Health and Care in the community.
What will a day in the life of a Service Coordinator entail?
- Participate in patient support planning at multi-disciplinary meetings in community venues and acute hospitals in Portsmouth.
- Promote the service and develop relationships with health and care partners to generate referrals through outreach and engagement.
- Triage referrals according to local service criteria and allocate cases for support, such as urgent welfare checks, transport, or food provision, within the required timeframe.
- Ensure that the impact of our service for service users is recorded through appropriate records (such as BRM), complete reports and paperwork as required.
- Hold line management responsibility for the support workers, casual support workers and volunteers.
- Be a contingency for other staff during periods of absence.
What do you need to be a successful Service Coordinator?
- Strong leadership skills and a knack to get the best out of people.
- A good communicator?
- Previous experience of Health and Social Care services in the community.
- Ability to travel across the area.
