Location: Covering Kensington & Chelsea, London
Salary: £30,000 per annum (plus ILW, if residing & working in London)
Hours: 35 per week

Join our pioneering NHS programme, High Intensity Use (HIU) Programme as an Outreach Caseworker, supporting vulnerable individuals across Kensington & Chelsea. This is your chance to be part of an innovative, compassionate initiative that’s transforming care for frequent users of urgent and emergency services. You’ll support individuals who frequently access services, empowering them to take control, avoid crisis, and reconnect with their communities.
What does a typical day as an Outreach Caseworker involve?
- Supporting individuals with complex needs to access the right help and avoid crisis.
- Identifying social factors driving high use of emergency services.
- Building trust with people facing challenges like trauma, addiction, and mental health issues.
- Reducing stigma and shifting behaviours through community engagement.
- Working closely with partners like A&E, GPs, housing teams, and mental health services.
What will you bring to the role of Outreach Caseworker?
- Experience supporting vulnerable people with complex needs, e.g. substance abuse, mental health conditions.
- Strong communication skills and the ability to build trust.
- Confidence in delivering casework, person-centred planning and risk assessment.
- Quick thinking and effective negotiation in fast-paced settings.
- Accurate record-keeping and attention to detail.
